- Fill sample bottle to no more than ¾ full
- Install cap until completely tight
- Fill out sample information form completely
- Place bottle in Ziploc bag, remove air and seal
- Place bottle/bag and sample information from inside mailer bag
- Peel off sealed liner, fold over and seal
- Drop off at any USPS pickup location
